First, take the foil cutter blade, and slide it all the way around the neck of the bottle. Remove the foil. Next, insert the sharp end of the worm into the center of the cork. Firmly grasp the bottle in your free hand, and twist in a clockwise motion. Stop once you've fully inserted the worm into the cork.
Now, place the shorter notch of the lever on the lip of the bottle. Pull the handle upwards. The cork will begin to lift out of the bottle. Finally, twist the cork off the wine bottle, and you're done!
